STEP 1: Referral
Texas law requires a prescription - provided by a referring practitioner* – in order to be treated for physical therapy services** in our clinic. For your convenience, you may have the ordering provider simply fax your prescription to our office along with your contact and insurance information to 512-345-6150. If you already have a prescription, please bring it with you to your first visit. A prescription is required prior to obtaining an appointment per our company policy.
*”Referring practitioner” means a qualified licensed health care professional who, within the scope of professional licensure, may refer a person for health care services. The term includes:
(A) a physician licensed to practice medicine by a state board of medical examiners;
(B) a dentist licensed by a state board of dental examiners;
(C) a chiropractor licensed by a state board of chiropractic examiners;
(D) a podiatrist licensed by a state board of podiatric medical examiners.
Other recognized referring providers include psychologists, nurse practitioners and optometrists.
For insurance purposes, a referral (aka: a pre-certification request) may be required by your insurance company prior to your first visit. Please review your insurance policy and/or call your insurance provider to confirm whether you require a pre-certification. This is of particular importance with Aetna plans.
**Diagnostic testing and comprehensive hearing tests do not require physician referral by law but your insurance company may require the referral. Please note that all Medicare plans require a referral for testing and therapy services. Hearing aid services do not require a referral and only a few insurance providers offer a hearing aid benefit.
STEP 2: Scheduling
Once a prescription is obtained simply call 512-345-4664 to make an appointment. Please be prepared to provide your contact and insurance information as well as the name of the ordering physician. If you have a prescription faxed to us, we will call you upon receipt of your prescription promptly to schedule your appointment.
If you have copies of other test results please bring with you or ask that the information be faxed to us at 512-345-6150 by your referring or other health care provider PRIOR to your appointment. Reviewing your medical records prior to your visit can be extremely valuable.
STEP 3: Insurance
We are a Medicare participating provider and we are contracted with most commercial insurance plans, excluding American Health Specialists (ASH) which is the 3rd party insurance provider that covers physical therapy services for Cigna. We accept Cigna for audiology testing only due to the poor reimbursement rates for the hands-on and one-on-one physical therapy we provide.
As a courtesy, we will obtain a quote of your benefits on your behalf prior to your first appointment. We do not guarantee the verbal quotes that we receive as stated in our quote of insurance benefits policy and we will collect fees according to what your insurance company tells us to on the date of your service. We encourage you to personally contact your insurance company directly to inquire about coverage and obtain a quote of what you may be expected to pay on the date of service. STEP 4: Payment
Co-payments, co-percentage payments, deductible payments, and private-pay payments are collected in full on the date of service and claims will be filed accordingly with your insurance company in a timely manner. Payment arrangement may be considered on a case by case basis and need to be arranged prior to your visit; (please contact our office to inquire).
NOTE (non-medicare patients): If you have a secondary form of insurance, we collect whatever your primary insurance company quotes us. We will file with both carriers, but we do collect what your primary insurance company quotes to us on the date of service. We accept cash, checks, and all major credit cards.
How to debate an invoice: If you receive an invoice from 360 Balance & Hearing you will need to debate any non-covered service(s) and/or “mis-quoted” benefits with your insurance company directly at the phone number listed on the back of your insurance card.
Note: Coverage and benefits quoted to 360 Balance & Hearing by your insurance company is not a guarantee of payment or coverage. This disclaimer is noted to us by all insurance carriers at the time of benefit checks to 360 Balance & Hearing. Also, it is important to note that once claims are filed it may take up to 90 days for your insurance company to process them.
We also provide financing services for hearing aid purchases through AllWell Financing, which is the premier vendor for select healthcare patient financing throughout the United States. AllWell offers low APRs and the loans are “installment loans” -- not a revolving line of credit. STEP 5: Your First Appointment

The initial visit will last no more than 1 hour for a physical therapy visit but may be longer for audiology/diagnostic testing. Frequency and duration of potential return visits will be determined at the time of your initial evaluation. We recommend wearing comfortable clothing and shoes. Workout clothes aren’t necessary however, you may wear them if you wish.
Please arrive fifteen minutes prior to your appointment to allow for necessary paperwork. If you are having testing done, please refrain from taking “anti-dizziness” medications, such as meclizine, antivert, valium, etc., at least 24 hours prior to your appointment. Bring a jacket or sweater as the room tends to get cold.
