Please print and complete the patient registration forms listed below. Kindly bring the forms to your appointment along with your insurance card(s) and photo ID.
IMPORTANT!
Please arrive a few minutes prior to your appointment to allow for necessary paperwork. If you are having testing done, please refrain from taking “anti-dizziness” medications, such as meclizine, antivert, valium, etc., 24 hours prior to your appointment. Bring a jacket or sweater as the room tends to get cold.
Our office is located at 7900 Shoal Creek Blvd., at the corner of Shoal Creek Blvd. and Anderson Lane. We occupy the second floor of the Bank of America building. Elevator access is available on the south side of the building, facing Anderson Lane. Handicap parking, however, is more accessible on the north side of the building. If your appointment is between 9:00am – 5:00pm, you may access the elevators by walking through the bank. Otherwise, you will have to walk around the east side of the building.
